How To Submit Your Online Waiver and Medical Form

Online registration/informed consent/medical form instructions

The first rule is, if you have any problems with online registration, feel free to call Kinesiology Client Services 403-220-7749 at any time, to get help.

1. To register your child for a program, you'll need to log into our registration system, and create a profile. If you already have an active living account Log in to your account first.   If you don't have an account simpley click the "create an account" button and follow the steps.
  • If you are creating a new account, your temporary log-in will be your date of birth entered year, month, day - YYYYMODY, no slashes or spaces - with the password that you'll create.
  • If you've forgotten your password, don't worry, simply click on the "trouble logging in" link and follow the instructions.
2. Once you've created or logged into your account, you'll need to add your family members. If you've done this step already, simply put in the family member (or dependent's) Alberta Health Care number and click the blue edit box to the right.  To add a family member, just click the box that says "Add Family Member" at the bottom of the page and follow the steps.
3. You are now ready to add a camp or camps to your shopping cart.
4. It's important that we know about any medical conditions, and have a physician contact number in case of emergency, so we require a medical form to be completed. If you haven't created one for a family member, click on the green box (see below for illustration) and fill out the form. If you have filled out a medical form previously, please check to make sure it is updated with correct contact and medical information.
 

Online Informed Consent Instructions

1. Once the medical form is added or updated and you have selected the camp or camps you want, click on “ My Events” at the top of the screen for a list of the camps.
2. Choose the participant using the drop down menu. All courses for that participant will appear, if an informed consent is required it will say “outstanding”.
3. Please click “sign waiver” button to sign the form.
