Informed Consent & Medical Information

Every camp program requires an online informed consent and completed medical information form. This form form must be filled out for every camp - even if the child is registered in multiple programs over the summer. You may be asked to complete this form at the time of an online registration, or follow the detailed directions that are outlined on your confirmation of registration.

Every Camp programs require a completed and updated Online Medical Informationform. Even if you have completed this form in the past, it is yourresponsibility to ensure it is updated with accurate medical and emergencycontact information.

These forms must becompleted before check-in on the first day of camp. If you fail to do so, itwill significantly delay your child's participation in the camp program.

Q. Can UCalgary Camps accommodate special needs students?

Please note that the University of Calgary will attempt to accommodate any students with special needs, however, given our limited resources, the University is not in a position to absorb the additional costs, if any, associated with this accommodation. Any student requiring accommodation should notify the camp co-ordinator well in advance with any special needs requests.

Q. Where do I go on the first day of camp?
